About idemidem
idemidem provides independent, person-centered transition support for individuals facing complex discharge barriers — including housing instability, behavioral health needs, and limited access to resources.
We work alongside hospital case management, discharge planning, and clinical teams — never in place of them. Our role is to strengthen the bridge between clinical discharge and real-world stability: connecting people to community resources, reinforcing care plans in plain language, and following up after discharge to help support lasting engagement.
Our Approach
- Independent of clinical decision-making — we complement, not replace, the care team
- Focused on practical, real-world barriers to safe discharge
- Committed to plain-language communication and follow-through
- Connected to a wide network of community and social service resources
Important Note
idemidem is not a medical provider and does not make clinical decisions. Medication management, crisis response, and treatment decisions remain the responsibility of the patient’s licensed care team. Availability of specific resources depends on eligibility, location, and program capacity.
